２.　本名簿に掲載している医療機関に、初めて診療に行く際には、極力、事前にその医療機関に連絡していただきますようお願いします。

If you are visiting any of the medical facilities on the list, please do your best to contact the facility beforehand.
Please try to bring along someone who can speak Japanese whenever possible.
It may be difficult to arrange foreign language support out of hours and on national holidays. When this is the case, please ensure that you bring someone who can speak Japanese with you.
